We offer two types of forms to suit your business needs:
1. Quick Connect Form
Capture more leads with our Quick Connect form - just the essentials to get started!
Includes:
- Basic customer information
- Basic pet information

2. Complete Enrollment Form
Get comprehensive client information with our Complete Enrollment form.
Includes:
- All customer information
- All pet information
- Your custom pet questions
- Your custom customer questions
Ideal for: Detailed information gathering and full client onboarding
How it works
1. Access the forms:
- Navigate to left side menu > communication > marketing
- Click on "Preview" to see how your forms look
- Click on "Copy Code" to get the embed code for your website (or social media platforms)
2. Client submission process:
- When a client fills out the form, you'll receive an immediate email notification
- Click the 'Click here' link in the email to view the client and pet profile details in PawsAdmin
3. Optional next steps:
- If the client is a good fit for your facility, you can invite them to claim their client portal account
- Once claimed, clients can fill in additional details and make online booking requests
Configuring Your Lead Capture Forms
Customize your forms to fit your specific business needs:
1. Navigate to: Left side menu > Admin > Configure Fields
2. Modify settings for default fields:
- Set as required or optional
- Mark as internal-only if needed
- Add custom questions/fields for both pets and customers
